Our new low vision brochure is available on request. We provide this free of charge to patients in Indiana, Ohio, Michigan, Kentucky and Illinois.

If you live outside those states,  please provide a stamped  self addressed envelope and $ 1.00 to cover handling and printing. Referring doctors and therapists may request a supply at no cost.
